Elder Services Unit

The staff provides community-based case management, which includes assessment for state-subsidized home care programs, implementation and maintenance of a care plan according to the client's needs and choices in compliance with The Department of Elderly Affairs Home and Community Care Program. The program provides clients with options to maintain independence through the implementation of services in the home.

Under contract to the RI Department of Elderly Affairs, the unit provides assessment, monitoring and other protective services for at risk clients over the age of 60.

In addition, a Registered Nurse provides ongoing support for clients and their caregivers.

An additional case management program, sponsored by the Tufts Health Plan Foundation provides education, support and safety equipment for clients in need.

Program eligibility varies. Please contact the unit for specifics.

Family Case Management

The Family Case Management Unit (City of Warwick, Certified Lead Center, Youth Success, and Supportive Housing) provides a variety of services to families and individuals in need of support services. Using a strengths-based perspective, case managers work to educate and empower clients to achieve positive outcomes.
