Elder & Family Case Management Services:
The Elder Services Unit (ESU) assists older
residents of Kent County with a variety of programs
including:
Co-Pay Program: Client (age 65 and over, meet
annual income guidelines) pays either $3 or $5.50
per hour for services of a certified nursing
assistant.
Waiver Program: Medicaid eligible client with
nursing home level of care may receive home based
services.
Assisted Living Case Management: Medicaid eligible
clients may receive subsidized care in local
assisted living facilities.
Protective Services: Case managers provide
supportive services for older residents referred
through Department of Elderly Affairs.
General Assessment: Complete needs assessment and
assistance for client and/or caregiver to develop an
effective plan of care.

Sustainable Options For Seniors
Funded through a grant from the United Way of RI,
the case management program focuses on Kent County
elders who may be experiencing difficulty in
remaining in their own homes. The case manager will
provide on-on-one counseling which specifically
addresses financial issues and home maintenance.
